Life Coaching Agreement and Disclosure

 

This Life Coaching Agreement (“Agreement”) is entered into by and between Jessica Barajas and Joe Macias (the “Coaches”) and [Client’s Full Name] (the “Client”). The coaching sessions will be held at Real Life Church, which provides space but does not offer, endorse, or assume any responsibility for the coaching services.

 

The Coaches agree to provide life coaching services designed to support and enhance the Client’s personal growth, development, and fulfillment. Coaching is a partnership between the Coaches and the Client to help the Client identify personal goals and develop actionable plans to achieve them.

 

Real Life Church is only a venue for the coaching sessions. The Church is not responsible for, nor does it provide, supervise, or endorse any coaching services. By signing this agreement, the Client acknowledges that Real Life Church holds no liability regarding the coaching sessions, and the relationship is solely between the Client and the Coaches.

 

Coaching is not therapy, counseling, or any other form of professional mental health care. The Coaches are not licensed mental health professionals, and coaching does not substitute for mental health services. The Client is responsible for seeking mental health or medical services as necessary from qualified professionals.

 

The Client agrees that they are fully responsible for their physical, mental, and emotional well-being during and after coaching sessions. All decisions and actions taken during and after coaching are the sole responsibility of the Client. The Coaches cannot be held liable for the Client’s outcomes, choices, or actions.

 

All personal information shared in the coaching sessions will remain confidential, except as required by law, such as if there is a perceived threat of harm to the Client or others. No information will be shared without the Client’s express written consent.

 

The Client agrees that the Coaches, Jessica Barajas and Joe Macias, as well as Real Life Church, will not be liable for any indirect, incidental, consequential, or special damages arising out of the coaching sessions. This includes, but is not limited to, loss of income, emotional distress, or other damages. The Coaches and Real Life Church are not liable for any actions taken by the Client based on coaching guidance.

 

Either party may terminate this Agreement at any time, with written notice. Real Life Church holds no involvement in such termination.

 

The Client understands that life coaching is not a substitute for legal, medical, or financial advice. The Coaches do not provide any such professional services, and the Client should consult appropriate professionals in those areas as needed.

 

This Agreement will be governed by and construed in accordance with the laws of the state of [Your State], without regard to its conflict of law principles.

 

By signing up below, the Client acknowledges that they have read, understood, and agreed to the terms of this Life Coaching Agreement.
